How to File a Medicare Claim By Mail

You will likely need to file a Medicare claim if you get care from a doctor who does not accept Medicare. This will increase your chances of receiving reimbursement for your care. You have up to 12 months after the date of your health care service to file the claim. 

Follow these steps to file a Medicare claim by mail: 

1. Find the CMS-1490S form. All claims use the CMS-1490S form. You can download the form from the Medicare website or have it mailed directly to your home by Medicare.
2. Complete the CMS-1490S form. Make sure all information is accurate and complete. You will need to include the following information on the claim:
   • Your personal information (name, Medicare number, address)
   • Details about the healthcare provider (name, address, national provider number)
   • Date of service
   • Description of the services you received
   • The amount charged for each service or item and the amount you paid
3. Attach any supporting documents. Include the original itemized receipts from the healthcare provider. Itemized receipts display the date of service, the specific service provided, and the associated costs.
4. Review your form for accuracy and completeness, and then send it via mail. Look up your state’s address to send Medicare claims and submit your completed CMS-1490S form and all supporting documents.

After processing, Medicare will send a Medicare Summary Notice (MSN). The MSN will indicate how Medicare processed your claim and the amount reimbursed for your care.
